Forms for Districts

These forms are for conservation districts to provide information to PACD.

PACD Delegate Designation Form
This form should be used by conservation districts to comply with PACD Bylaws, Article V, Section II relating to voting delegates. PACD delegate information does not need to be updated annually. Delegate information should be updated on an on-going basis as changes occur.

State Legislative Contact Form
Each conservation district should select a director or associate director who is willing to receive legislative updates and action alerts and take appropriate action on behalf of the conservation district board. (To ensure that no district is without a state legislative contact, PACD will list the conservation district board chair as the default contact if the name and contact information for another director or associate director is not submitted.)

Committee Interest Form
Interested district directors, associate directors, and staff should complete and submit a Committee Interest Form to be considered for appointment to a PACD committee. If there is no vacancy on the committee(s) indicated, individuals who submit an interest form will be considered as openings arise. PACD Listserves:
PACD maintains several electronic mailing lists called "listserves" as a free service to the conservation districts.

Conservation District Directors
A listserve has been created specifically for directors. Since directors and associate directors change each year, a new form must be completed by the district each year. Please complete one form per county to subscribe directors to the listserve.

Conservation District Managers
A listserve has been created specifically for district managers and assistant managers (if applicable). Please complete one form per county as needed.

Conservation District Environmental Education Coordinators
A listserve has been created specifically for conservation district environmental education coordinators. Please complete one form per county as needed.

Conservation District Watershed Specialists
A listserve has been created specifically for conservation district watershed specialists. Please complete one form per county as needed.
